Long Term Acute Care Hospitals - LTACHS
What is a long-term acute care hospital? It is a special classification of hospital recognized by the federal government for facilities that meet required specifications, including maintenance of an average length of stay of at least 25 days per patient and meeting all conditions of participation as an acute care hospital with regard to administration and basic hospital functions. Patients that are admitted to LTACHs still require acute care but on a longer term basis than an acute care hospital can provide. LTACHs bridge the gap between Acute Care Hospital Care and Home or Rehab.

Complex Post-surgical Conditions
Hem dialysis/ Peritoneal Dialysis
Long-term IV Antibiotic Therapy
Wound Care Needs
Rehabilitation Capability with Complicated Needs
PT/OT and Speech Therapy Needs
Nutritional counseling
While under the care of an LTACH a professional case manager will be available to: Serve as the communication link between you, your family, your physician and the staff.Work with you and the interdisciplinary team to enhance your care and your overall satisfaction while in the hospital.Identify obstacles and conflicts with your care delivery and resolves them with the interdisciplinary team.Coordinate care plan meetings to outline your specific care needs and your progress towards discharge.Facilitate and attains insurance approval and certification for your inpatient stay at CCH.Help coordinate you hospital length-of-stay with your physician and insurer.Coordinate transfer to other facilities as needed or coordinates your discharge to your home.Coordinate Home Health and Home Care Services and necessary medical equipment.Utilize community resources for individual needs.
