Christian Health Center is nestled between historic Old Louisville and downtown, and is easy to get to by interstate and bus routes. Our Transitional Rehab-to-Home unit provides:
• Spacious private and semi-private rooms with home-like amenities
• Short-term physical, occupational and speech therapy
• Skilled nursing care
• Patient education
• Social services
Our goal is to help patients recuperate, gain strength and return home as quickly as possible! Until that goal is met, we offer vibrant activity programs, delicious meals, and a full-time chaplain to help make the temporary stay a great one. We accept Medicare, Medicaid and private insurance.
In addition to Transitional Rehab-to-Home care, Christian Health Center provides specialized memory care, long-term skilled nursing care and outpatient rehab services.
